Skin Tightening and Lifting

Skin Tightening and Lifting
Skin loses firmness gradually, so most people notice the result before they notice the process. The jawline softens, cheeks flatten a little, and the neck starts to look less defined than it used to. At Hawaii Dermatology & Surgery, we offer non-surgical options that address both sides of that change, restoring lost volume with dermal fillers and rebuilding the skin's own support structure with XERF radiofrequency treatment. Patients across Oahu visit our Aiea and Kailua offices for results that look natural and require no recovery time.

What Is Skin Tightening and Lifting?
Firmness in the face and body depends on two things working together, the volume underneath and the collagen that holds everything in place. Both decline with age, sun exposure, and weight change. Skin tightening and lifting treatments address these without surgery, either by replacing volume directly or by encouraging your own tissue to rebuild. Many patients get the best outcome by combining the two approaches, and our team will explain what may be a good fit for what you want to change.
Dermal Fillers
Dermal fillers are injectable treatments that restore volume where it has been lost and add structure where more definition would help. As the face ages, fat pads shift and shrink, which is what creates deep folds, hollow tear troughs, flattened cheeks, and jowls along the jawline. Filler placed carefully in these areas replaces that support and lifts the tissue above it.
Fillers can also be used to shape rather than simply restore. Common goals include a sharper jawline, more projected cheekbones, fuller lips, and smoothing of the deep creases running from the nose to the corners of the mouth. Results are visible immediately, treatment takes very little time, and there is minimal downtime beyond possible mild swelling or bruising for a few days.
XERF Radiofrequency Treatment
XERF is an FDA-cleared, non-invasive radiofrequency treatment from Cynosure Lutronic, and it takes a different approach to lifting. Rather than adding anything, it delivers controlled thermal energy beneath the surface of the skin to contract existing collagen and stimulate the fibroblasts responsible for producing new collagen and elastin.
What makes XERF different from earlier radiofrequency devices is that it uses two frequencies rather than one, 6.78 MHz and 2 MHz, delivered in the same session. That lets treatment reach shallow, middle, and deeper layers of tissue together, including the deeper structural layer that has so much to do with how lifted and defined the face and neck appear. Built-in cooling and real time temperature monitoring keep the session comfortable, and because radiofrequency does not target pigment, XERF is appropriate across a wide range of skin tones.
Areas We Treat and What to Expect
These treatments can be used on the face, neck, jawline, and areas of the body where skin has lost elasticity. XERF sessions typically run twenty to forty minutes depending on the area, with no needles, no numbing, and no downtime afterward. Some tightening is often noticeable right away, though the real change builds over the following two to three months as new collagen develops. Filler results, by contrast, are visible the same day. Your consultation will cover which approach may suit your skin, whether combining them makes sense, and how many sessions to expect.

FAQ
FAQs
Answers to your frequently asked questions.
What is the difference between dermal fillers and XERF?
They work on different problems, which is why they pair so well. Fillers replace volume that has been lost, restoring support and shape right away. XERF works on the quality of the skin itself, tightening what is there and prompting your body to build new collagen over the following months.
Is there any downtime with XERF?
No. XERF is a non-invasive treatment with no needles and no incisions, and most patients go straight back to their day afterward. Skin may look slightly flushed or feel warm for a short while, but there is no recovery period to plan around.
Is XERF safe for all skin tones?
Because XERF uses radiofrequency rather than light, it does not target pigment in the skin. That makes it a safe option across a wide range of skin tones, including deeper tones where certain laser treatments require more caution.
How soon will I see results from XERF?
Many patients notice a subtle tightening effect soon after their first session. The more meaningful change builds gradually over the following two to three months as new collagen forms, and additional sessions may be recommended depending on your starting point and goals.
